VBS Registration

As a parent/guardian I am giving MPBC my permission for my child/children to participate in the 2019 Vacation Bible School (VBS). With this permission I understand the risks and hazards incidental with a VBS event. I have given my child/children instruction to obey all the rules given to the children by the MPBC volunteers and supervisors. MPBC takes photographs of participating children to display as an overview of the VBS week with our church.
